Dumb question (samples?)

I think this may be a dumb question but here it goes anyway.
In the teacher book there are reproducible CD Exercise Drill sheets. I get the Vocabulary and Latin Saying Sheet. It is the Grammar Form I am not clear on how to use. Is there a sample form somewhere? I need a visual. It is probably not as difficult as I am making it out to be, but I have a mental block right now. Do you copy 5 sheets a week or use 1 per week? Ok, that is 2 dumb questions. :)
